DevotedDOC is a physician-founded, physician-led multi-state medical group providing telemedicine-based care. We are committed to protecting the privacy of your health and personal information and maintaining compliance with all applicable privacy and data security laws, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

We collect personal identifiable information (PII) such as your name, address, contact details, and date of birth. We also collect protected health information (PHI), including medical history, current medications, treatment plans, laboratory results, and other health-related data necessary for medical care. Technical data such as IP address, device identifiers, browser type, and access logs may be collected when you use our site. We may also collect billing and payment information for services rendered.

Your information is used for the purpose of delivering healthcare services, communicating with you about your care or our services, processing payments, facilitating prescriptions and lab testing, and for internal operations such as quality improvement, training, and compliance monitoring. DevotedDOC is not a substitute for emergency medical care. If you are experiencing a medical emergency, you must call 911 or visit your nearest emergency department.

You must be at least eighteen (18) years old or have parental consent to use our Services. You agree to provide accurate and complete information when using our platform and not to engage in any unlawful or abusive activity on our site or with our staff. All content on our website, including branding, text, logos, and medical materials, is owned by DevotedDOC and may not be copied or distributed without written permission.

DevotedDOC services are provided on a cash-pay basis. You agree to pay all applicable fees in full at the time of service. No refunds are issued for completed medical consultations. Subscription services may be canceled by notifying us prior to the next billing cycle.

You understand and agree that DevotedDOC is not liable for damages arising from your use or inability to use our website or telemedicine platform, including technical failures, delays, or treatment outcomes. We make no guarantee regarding the effectiveness or appropriateness of any prescribed treatment.

We reserve the right to amend this Agreement at any time. Any updates will be posted with a revised effective date and will become effective upon posting. Continued use of the Services after such changes constitutes your agreement to the revised terms.

This Agreement shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law principles. If any provision herein is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

COMPOUNDED MEDICATION DISCLOSURE
Compounded medication contains Tirzepatide or Semaglutide combined with vitamin B12. It is specifically formulated for individual patients who may not tolerate standard formulations or who require a customized therapeutic approach. This medication is prepared by a licensed compounding pharmacy in accordance with Section 503A of the Federal Food, Drug, and Cosmetic Act and is dispensed only pursuant to a valid, patient-specific prescription from a licensed healthcare provider.

Our compounded formulation is not affiliated with, endorsed by, or a substitute for the FDA-approved Tirzepatide product manufactured by Eli Lilly and Company. The inclusion of glycine and vitamin B12 is intended to support patients experiencing fatigue, muscle loss, or neuropathy—common during weight loss or diabetes management—and may provide additional metabolic, neurological, or musculoskeletal benefits.

This compounded medication is not for resale, bulk distribution, or office use. It is intended solely for individual patients when a healthcare provider determines that commercially available alternatives are not appropriate to meet the patient's specific clinical needs.
