Your treatment continues beyond the injection
VIVITROL is used for ongoing alcohol use disorder treatment. It does not treat acute alcohol withdrawal. If withdrawal risk is identified during your evaluation, your clinician will discuss the safest next step and whether a different level of care is needed before outpatient treatment.
More of your care from home. In-person care when you need it.
Private care with fewer unnecessary barriers
Telemedicine allows many patients to receive ongoing alcohol-treatment care privately without repeated visits to a traditional treatment setting. When an in-person service is needed, such as laboratory testing or a VIVITROL injection, we help patients understand their local options.
Alcohol use disorder deserves the same privacy, respect, and medical professionalism as any other health condition.
Specimen collection is performed by the laboratory or an independent mobile collection provider where available. Local administration sites may be independent healthcare organizations and are not DevotedDOc offices unless specifically identified.
The medication
What is VIVITROL?
VIVITROL is extended-release naltrexone, a non-opioid medication FDA-approved for the treatment of alcohol dependence as part of a comprehensive treatment program. It is given by a healthcare professional as a 380 mg intramuscular injection approximately once every four weeks or monthly.
Patients should not be actively drinking at the time of their first VIVITROL injection and should be able to abstain from alcohol before treatment begins.
For some patients, injectable naltrexone can help reduce alcohol craving and heavy drinking.
Monthly treatment
No daily tablet to remember.
Non-opioid
Naltrexone is not an opioid and is not physically addictive.
Can reduce cravings and heavy drinking
For appropriate patients, as part of treatment.
Part of an ongoing treatment plan
Used as part of a comprehensive treatment plan that includes medical follow-up and psychosocial or recovery support.
Oral naltrexone vs. VIVITROL
Oral naltrexone
- Taken by mouth, usually daily
- Filled through a pharmacy
- Can be adjusted or discontinued more readily
- Useful for many patients
Monthly VIVITROL injection
- Injection approximately once monthly
- Given by a healthcare professional
- Avoids daily pill-taking
- Remains active for an extended period after administration
Important before VIVITROL
Tell your clinician about any recent opioid use
VIVITROL blocks opioid receptors. If opioids are still in your body or you are physically dependent on opioids, receiving VIVITROL can cause sudden and severe opioid withdrawal. Patients generally need to be opioid-free for at least 7–10 days before starting VIVITROL. People transitioning from buprenorphine or methadone may require a longer interval. Your clinician will determine when it is safe to begin treatment.
Your clinician will review recent opioid exposure and determine whether VIVITROL is appropriate and when treatment may safely begin.
VIVITROL can interfere with opioid pain medications. Tell your surgeon, dentist, anesthesia team, or emergency clinician that you receive VIVITROL and provide the date of your most recent injection. For planned procedures, discuss pain-control options with the treating surgical or anesthesia team in advance.

Progress can look different for different people
VIVITROL can be part of a treatment plan designed around your health, drinking pattern, and recovery goals.
Some patients want to reduce cravings and heavy-drinking days. Others are working toward complete abstinence. For many people, goals change over time as treatment progresses.
What matters is having a plan that is medically appropriate, realistic, and supported by ongoing care.
Your DevotedDOc clinician will work with you to review progress, adjust treatment when needed, and help determine the next step.
One plan, reviewed over time
- Reduce cravings
- Reduce heavy drinking
- Support recovery
VIVITROL is not right for everyone
Your clinician should know about:
- recent opioid use
- opioid dependence or withdrawal
- liver disease
- pregnancy
- serious medication allergy
- planned procedures where opioid pain medication may be needed
- previous serious injection-site reactions
Trying to overcome VIVITROL's opioid blockade with large amounts of opioids can cause overdose, coma, or death.
Opioid tolerance may be lower when VIVITROL wears off, after a missed injection, or after treatment ends. Previously tolerated opioid doses can become dangerous.
Contact your administration site or DevotedDOc for significant injection-site pain, increasing redness or swelling, drainage, blistering, or an open wound.

What is VIVITROL used for?
VIVITROL is used as part of treatment for alcohol use disorder. It also has a separate FDA-approved use in opioid use disorder; this page focuses on alcohol use disorder.
How does VIVITROL work for alcohol use disorder?
VIVITROL contains naltrexone, which blocks opioid receptors involved in some of alcohol's rewarding effects. For some patients, this can help reduce craving and heavy drinking. It is not a cure and is used as part of an ongoing treatment plan.
How often do you get a VIVITROL injection?
The recommended dose is 380 mg, given as a deep intramuscular injection in the buttock muscle approximately every four weeks or once a month. Each injection is administered by a healthcare professional.
Can I get VIVITROL through telehealth?
Your evaluation, treatment planning, and follow-up can take place through DevotedDOc telehealth. The injection itself is administered in person by an appropriate healthcare professional at a qualified local administration site.
Where can I get a VIVITROL injection near me?
Depending on your location and availability, options may include medical clinics, community or nonprofit health centers, injection or infusion centers, and qualified pharmacy-based administration sites where permitted. Use the state map or ZIP search to explore currently identified options. You choose from available appropriate sites.
Find Local VIVITROL Injection AccessDoes DevotedDOc give the injection?
Generally no, unless a location is specifically identified as DevotedDOc. DevotedDOc provides your ongoing medical care and helps you identify qualified local administration options, which are typically independent healthcare organizations.
Do I need oral naltrexone before VIVITROL?
No. Pretreatment with oral naltrexone is not required, although it may sometimes be used based on patient preference or clinical judgment.
Can you drink while taking VIVITROL?
VIVITROL is not disulfiram (Antabuse) and does not intentionally make you sick if you drink alcohol. However, VIVITROL does not make heavy drinking safe and does not treat alcohol withdrawal. Patients should not be actively drinking at the time of their first VIVITROL injection and should be able to abstain from alcohol before treatment begins. Your DevotedDOc clinician will review your drinking pattern, withdrawal risk, and treatment goals to determine when treatment can safely begin.
How long do I need to be opioid-free before starting VIVITROL?
Patients generally need to be opioid-free for at least 7–10 days before starting VIVITROL to reduce the risk of sudden opioid withdrawal. People transitioning from buprenorphine or methadone may require a longer opioid-free period. Your DevotedDOc clinician will review your recent opioid use and determine when VIVITROL can safely be started.
Does VIVITROL help with alcohol withdrawal?
No. VIVITROL is used for ongoing treatment of alcohol use disorder and does not treat acute alcohol withdrawal or replace medically supervised detoxification. If withdrawal risk is identified during your evaluation, your DevotedDOc clinician will discuss the safest next step, which may include supervised or in-person care before outpatient treatment begins.
Can I take opioid pain medication while on VIVITROL?
VIVITROL blocks opioid receptors, so usual doses of opioid pain medication may not work while it is active. Never try to overcome the blockade by taking more opioids; this can cause overdose, coma, or death. Anyone treating your pain needs to know you receive VIVITROL and when your last injection was.
What if I need surgery while receiving VIVITROL?
Tell your surgeon, dentist, anesthesia team, or emergency clinician that you receive VIVITROL and the date of your most recent injection. For planned procedures, discuss pain-control options with the surgical or anesthesia team in advance, and let your DevotedDOc clinician know about upcoming procedures.
What are common VIVITROL side effects?
Commonly reported side effects include nausea, headache, dizziness, tiredness, trouble sleeping, decreased appetite, muscle cramps or joint pain, and injection-site tenderness. Less common but serious risks include severe injection-site reactions, liver problems, depressed mood, and allergic reactions. Review the FDA Medication Guide and contact your clinician with concerns.
Read the FDA Medication GuideDoes insurance cover VIVITROL?
Coverage varies by plan and is never guaranteed. VIVITROL may be covered under your medical or pharmacy benefit, and some plans require prior authorization. The administration site or pharmacy generally handles benefits verification and authorization, and your health plan makes final coverage decisions.
How much does DevotedDOc treatment cost?
DevotedDOc medical visits are $99.99 per visit. Medication, laboratory testing, injection-site charges, pharmacy services, and other outside services are separate.
What other medications are available for alcohol use disorder?
Other FDA-approved medications for alcohol use disorder include oral naltrexone, acamprosate, and disulfiram. DevotedDOc evaluates patients for appropriate options after an individual medical evaluation.
