Medicare GLP-1 Bridge Program: Eligibility Explained
Wondering if you qualify for the Medicare GLP-1 Bridge Program? Schedule a consultation with DevotedDOc.
The Medicare GLP-1 Bridge Program is a new Medicare demonstration program that begins on July 1, 2026, to help eligible beneficiaries access certain GLP-1 medications for chronic weight management at a flat cost of $50 per month. The program was created to improve access to FDA-approved weight loss medications for people who meet specific Medicare coverage and clinical eligibility requirements.
Not everyone with Medicare qualifies for the program. Eligibility depends on several factors, including your Medicare prescription drug coverage, age, body mass index (BMI), qualifying medical conditions, and whether your current Medicare Part D plan already covers a GLP-1 medication. Before coverage is approved, your healthcare provider must also submit a prior authorization request confirming that you meet the program’s requirements. You can learn more about the program in the official Medicare GLP-1 Bridge fact sheet.
This guide explains who qualifies for the Medicare GLP-1 Bridge Program, the Medicare GLP-1 requirements you need to meet, and how to determine whether you’re eligible before applying.
What Is the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge Program is a temporary Centers for Medicare & Medicaid Services (CMS) demonstration that provides eligible Medicare beneficiaries with access to select GLP-1 medications used to reduce excess body weight and maintain weight loss. Unlike traditional Medicare Part D coverage, the program is specifically intended for qualifying individuals seeking treatment for obesity or overweight with certain weight-related health conditions.
Beginning July 1, 2026, eligible beneficiaries can receive one of the covered medications for $50 per month, regardless of income. At this time, the program includes:
- Foundayo® (tablet)
- Wegovy® (injection and tablet)
- Zepbound® KwikPen®
Not all formulations are covered. For example, the single-dose Zepbound pen and Zepbound vials are not included in the program. Additional eligibility information and provider guidance are available on the official CMS Medicare GLP-1 Bridge information page.
Who Qualifies for the Medicare GLP-1 Bridge Program?
The Medicare GLP-1 Bridge Program is available only to beneficiaries who meet specific coverage and clinical requirements established by the Centers for Medicare & Medicaid Services (CMS). To qualify, you must have eligible Medicare prescription drug coverage, meet the age requirement, satisfy one of the BMI eligibility pathways, and receive a prescription for an approved GLP-1 medication for weight management. Your healthcare provider must also verify that you meet all program requirements before submitting a prior authorization request.
You Must Have Eligible Medicare Prescription Drug Coverage
To participate in the Medicare GLP-1 Bridge Program, you must be enrolled in either:
- A standalone Medicare Part D Prescription Drug Plan (PDP), or
- A Medicare Advantage plan that includes prescription drug coverage (MA-PD).
Certain Medicare plan types are not eligible for the program unless they also include an eligible standalone Part D plan. If you’re unsure whether your current coverage qualifies, reviewing your Medicare plan details or contacting Medicare can help clarify your eligibility.
You Must Be at Least 18 Years Old
The program is available only to adults who are 18 years of age or older. However, meeting the age requirement alone does not guarantee eligibility—you must also satisfy the Medicare obesity eligibility criteria based on your BMI and health history.
You Must Meet Medicare BMI Requirements
CMS offers three different eligibility pathways based on your body mass index (BMI) and certain qualifying medical conditions.
BMI of 35 or Higher
Adults with a BMI of 35 or greater may qualify if the medication is prescribed to reduce excess body weight and maintain weight loss alongside ongoing lifestyle changes.
BMI of 30 or Higher With a Qualifying Medical Condition
You may also qualify with a BMI of at least 30 if you have one or more of the following:
- Heart failure with preserved ejection fraction
- Uncontrolled high blood pressure despite treatment with two antihypertensive medications
- Chronic kidney disease stage 3a or higher
BMI of 27 or Higher With Certain Cardiometabolic Conditions
Adults with a BMI of at least 27 may qualify if they have one or more of these conditions:
- Prediabetes
- Previous heart attack
- Previous stroke
- Symptomatic peripheral artery disease
If you’re wondering who qualifies for Medicare GLP-1, these BMI pathways are one of the most important parts of the eligibility review. If you have prediabetes, you can also learn more about Ozempic for prediabetes to understand how coverage may differ for other GLP-1 medications.
Who Does Not Qualify?
Even if you meet the BMI requirements, you may not be eligible for the Medicare GLP-1 Bridge Program if you don’t meet all of the Medicare GLP-1 requirements.
In general, you are not eligible if:
- Your Medicare Part D plan already covers a GLP-1 medication for an approved medical condition.
- You have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or metabolic dysfunction-associated steatohepatitis (MASH), as these conditions may already qualify for GLP-1 coverage through your Medicare drug plan.
- You are enrolled only in certain Medicare plan types that are excluded from the demonstration program.
If you’re considering another GLP-1 medication for weight management, you may also find it helpful to review the Zepbound eligibility criteria to better understand who qualifies.
How to Know If You Meet the Medicare GLP-1 Requirements
If you’re unsure whether you qualify for the Medicare GLP-1 Bridge Program, the best place to start is by reviewing the program’s eligibility criteria with your healthcare provider. They can evaluate your medical history, calculate your body mass index (BMI), and determine whether you meet the clinical requirements established by Medicare.
Before applying, consider the following questions:
- Do you have Medicare Part D prescription drug coverage through an eligible plan?
- Are you at least 18 years old?
- Do you meet one of the BMI eligibility pathways?
- Do you have any qualifying health conditions, such as prediabetes, chronic kidney disease, heart failure, or a history of cardiovascular disease?
- Does your Medicare Part D plan already cover a GLP-1 medication for another approved condition?
If you answer “yes” to these questions, you may be eligible for the Medicare GLP-1 Bridge Program. Your healthcare provider can help confirm your eligibility and determine whether a covered GLP-1 medication is appropriate for your weight management goals.
How to Apply for the Medicare GLP-1 Bridge Program
If you meet the eligibility requirements, enrolling in the program involves several steps. Medicare requires healthcare providers to submit documentation before coverage is approved.
Talk With Your Healthcare Provider
Your provider will review your BMI, medical history, current medications, and weight management goals to determine whether you qualify for the Medicare GLP-1 Bridge Program. They will also confirm that an eligible GLP-1 medication is appropriate for your situation.
Receive a Prescription
If you qualify, your provider will send a prescription for one of the covered GLP-1 medications to your pharmacy. The pharmacy may request your Medicare ID number to verify your eligibility before processing the prescription.
Complete Prior Authorization
After your pharmacy confirms your eligibility, your healthcare provider must submit a prior authorization request to Medicare. This documentation verifies that you meet the program’s Medicare GLP-1 requirements and that the medication is being prescribed for weight management alongside ongoing nutrition and physical activity.
Receive Medicare Approval
Once Medicare reviews the prior authorization request, you’ll receive a notice indicating whether your medication has been approved through the Medicare Bridge coverage program. If approved, your provider and pharmacy can move forward with filling your prescription.
Pick Up Your Medication
After approval, you can pick up your medication from your pharmacy and pay the program’s $50 monthly cost for a one-month supply. In most cases, you won’t need a new authorization for refills or dose adjustments as long as you remain on the same covered GLP-1 medication.
