Long-Acting Buprenorphine
Treatment in California
Buprenorphine is a medication used to treat opioid use disorder. It reduces cravings and withdrawal and is one of the most established treatments for OUD.
Traditional formulations such as films or tablets are usually taken every day. Extended-release formulations release buprenorphine over a longer period, which removes the daily dosing routine and the need to keep medication at home.
BRIXADI and SUBLOCADE are both extended-release buprenorphine formulations. Both are administered by a healthcare professional rather than self-injected. Whether either one is a good fit depends on your history, your current medication, and your clinical picture — that determination is made by your clinician.
BRIXADI®
Weekly and monthly long-acting buprenorphine options
BRIXADI is given as an injection under the skin on either a weekly or a monthly schedule, which gives your clinician more than one dosing rhythm to work with. It is administered by a healthcare professional at a clinic.
SUBLOCADE®
Monthly extended-release buprenorphine
SUBLOCADE is given as a monthly injection administered by a healthcare professional. One appointment a month covers your buprenorphine dosing for that month.

Does Insurance Cover BRIXADI
or SUBLOCADE in California?
Coverage varies by health plan, medication and administration location. Commercial insurance, Medicare and Medi-Cal may provide coverage for extended-release buprenorphine. Prior authorization, billing requirements and patient cost-sharing can vary, so benefits should be verified before an injection is scheduled.
How coverage generally works in California. Not a guarantee of coverage or payment.
| Coverage Type | What to Expect |
|---|---|
|
Commercial insurance
|
BRIXADI and SUBLOCADE may be covered under a patient's pharmacy or medical benefit, depending on the plan and administration site. Prior authorization or other utilization requirements may apply. Manufacturer copay-savings programs are available for some eligible commercially insured patients; patients enrolled in government-funded insurance programs are generally not eligible for those copay programs. |
|
Medicare & Medicare Advantage
|
Coverage depends on the patient's specific Medicare benefits, the medication and how the administering location bills for it. The receiving location should verify benefits, patient cost-sharing and any authorization requirements before scheduling. |
|
California Medi-Cal
|
Extended-release buprenorphine formulations corresponding to both SUBLOCADE and BRIXADI appear on California's Medi-Cal Rx Contract Drugs List. Coverage, utilization controls and billing requirements can still depend on how the medication is supplied and administered, so eligibility and requirements should be verified for each patient before treatment. |
|
Self-pay
|
Cash pricing varies by administration location. Patients should request an estimate for both the medication and administration before scheduling. Manufacturer patient-support resources may also be available depending on eligibility. |
Medication coverage and administration-site participation are two different things
A medication may be covered by your health plan even when a particular California pharmacy or injection site does not participate with that plan or cannot bill your specific benefit. DevotedDOc and the receiving administration site verify the applicable access pathway before treatment is scheduled.
If you have Medi-Cal
BRIXADI and SUBLOCADE may be covered through Medi-Cal depending on the patient's benefits, authorization requirements and how the medication is supplied and billed. However, not every participating BRIXADI administration site accepts Medi-Cal or is able to bill the medication and administration through the same benefit.
If you have Medi-Cal, contact DevotedDOc and we'll help you understand the available options and identify an appropriate administration pathway when available.
Medication coverage through Medi-Cal does not mean statewide injection access. Injection availability depends on medication availability, the administration site's participation with your plan and applicable authorization requirements.
Coverage information is educational and is not a guarantee of benefits, authorization or payment. Plan requirements and administration-site participation can change. Your benefits and applicable access requirements are verified before an injection is scheduled.
Why BRIXADI and SUBLOCADE Are Given by a Healthcare Professional
These are provider-administered medications. They are not dispensed to patients for self-injection, and patients do not take the medication home to administer themselves.
Both medications are subject to special distribution and administration requirements. The medication is supplied to an authorized healthcare setting or participating administration site, and a qualified healthcare professional gives the injection.
That is why the California model has two parts. A participating pharmacy, clinic or other authorized administration site handles the physical injection. DevotedDOc maintains the clinical treatment relationship — your evaluation, treatment plan, medication decisions and follow-up care.
It also means you do not have to manage the medication-supply process on your own. Depending on the location and insurance pathway, the receiving administration site may help coordinate benefits verification, prior authorization, medication acquisition and injection scheduling before your appointment.

Can I Switch From Suboxone® or Daily Buprenorphine to an Injection in California?
Many patients already taking daily buprenorphine may be candidates for a long-acting injection. It is one of the most common reasons patients explore BRIXADI or SUBLOCADE treatment with DevotedDOc.
Talk With an Addiction ClinicianBRIXADI and SUBLOCADE have different initiation requirements, including considerations related to your current buprenorphine treatment and clinical history.
Whether and when you stop taking daily films or tablets depends on the medication you and your clinician choose and your individualized treatment plan.
The transition depends on several clinical factors — including your current dose, treatment history, other medications and response to buprenorphine.
Your DevotedDOc clinician builds the transition plan with you. We do not provide individualized dosing instructions online, and you should not change or stop your medication without guidance from your treating clinician.

DevotedDOc also provides evaluation and ongoing treatment for alcohol use disorder, including extended-release naltrexone (Vivitrol®) when clinically appropriate.
Like long-acting buprenorphine, Vivitrol is administered by a healthcare professional at a local California administration site, while DevotedDOc can provide the clinical evaluation, treatment planning and ongoing follow-up through telemedicine.
